The RetroPerc procedure begins with the positioning of a flexible ureteroscope in a selected infundibulum with its tip positioned at the papilla for puncture.  The path of the flexible ureteroscope thereby matches the long axis of the infundibulum precisely in three dimensions (X-Y-Z axis). The puncture wire advances in this very same X-Y-Z alignment, followed by the coaxial exchange catheter, new 0.035” nephrostomy wire, balloon dilator and nephrostomy sheath.  The result of this series of exchanges is that the nephrostomy sheath is positioned in perfect 3-dimensional alignment with the infundibulum, thereby reducing renal trauma.
